Why a custom articulating limited ROM AFO is used for this pathology: Western Carolina O&P designs and fabricates this type of AFO specifically tailored for this common pathology for superior subtalar triplanar control and foot ankle alignment support. Our practitioners start with a comprehensive foot and ankle evaluation of the mechanical and fitting perimeters, and once defined, a design plan for the custom AFO is established. A negative mold impression, or digital scan is taken of the patient affected lower extremity so a custom AFO can be fabricated. Western Carolina O&P, with our state of-the-art fabrication lab, produces a structural supportive and comfortable device to support the proper foot structures, provide maximum hind foot stability and control to restore dynamic gait mechanics for safe ambulation and joint stability. This custom fabricate AFO, also known as a gauntlet style AFO, is designed to be light weight and strong. Because it is custom fabricated to a positive model all abnormal structural changes that usually accompany PTTD, AO, and Charcot can be accommodate. This type of AFO is designed to be a total contact fit with maximum joint and hind foot control to stabilize very unstable joint mechanics. Care is taken to achieve proper alignment so as not to jeopardize upper chain joint mechanics and provide a smooth line of progression. This type of AFO prevents motion both in the sagittal and coronal plane and will limit torque motion through the joint. This not only helps the patient with increased safe stable ambulation, but also helps to relieve pain and slow joint degradation that is caused by motion.
Orthotic specific design consideration: This custom AFO is designed and fabricated with a total contact inner leather boot with padding at all bony prominences and a carbon fiber infused plastic shell. Because the AFO is built from a modified positive mold of the patients’ lower extremity, a total contact device is produced. The total contact fit and use of rigid outer shell produces a comfortable and functional brace. This type of AFO can have several different special closer systems including lace, Velcro and BOA speed closers. The right closer system helps to properly secure the foot and ankle in the brace to provide maximum support.
